Dementia Clinical Nurse Specialist- Band 7
The Barnet Older People Service requires a Dementia Clinical Specialist Nurse with experience of working in Older Peoples Mental Health Services especially diagnosis and management dementia service users.
The Dementia Clinical Nurse Specialist (DNS) post will be fully integrated into Barnet’s Primary Care Network 5 and Community Services. The post holder is required to promote good dementia care and share their expertise with all primary care and community services staff to increase confidence knowledge, skills and understanding of dementia.
The post holder will work across organisational and professional boundaries, to bring about improvement in the care and treatment of Dementia for service users and families by working with the Barnet CMHT Team Manager, primary care, adult social care, Barnet’s Memory Service, the VCS and CCG in developing a more integrated and less fragmented approach to care that helps maintain patients physical and mental health and wellbeing, The post holder will be a key member of a small supportive team and will receive regular supervision and training, as required.
