An opportunity has risen for a suitable and Physiotherapist to join our renowned multi-professional INTERMEDIATE CARE TEAM in Enfield Community Services with a reputation in delivering excellence in Care.
We are currently looking for a highly motivated and passionate physiotherapist to join our multi-disciplinary Intermediate Care Team (ICT). The Team consists of Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurse, Support workers and technical Instructors.
The role will involve working with patients and other health care professionals to provide a holistic approach to patient care, thereby facilitating early discharge from hospital, prevent hospital admissions and aid patients to stay independent in their own home for as long as possible
The post holder will deliver a wide range of physiotherapy practice as part of the community based inter-professional team across the geographical areas covered by ICT in Enfield. This includes working with patients in their homes, in reaching into acute trusts, and supporting work streams for Early Supported Discharges from Older People Assessment Unit (OPAU), Post-Acute Care Enablement (PACE). The post holder strives to continually improve care pathways for patients, their carers and families when utilising services in conjunction with other statutory and voluntary services.
You will possess strong interpersonal skills and the ability to contribute in change management. In return the Trust will provide excellent support and encouragement for a personal and professional development.
